Fitness to Drive

Save time – have your Fitness to Drive medical assessment submitted online

If you’re an older driver, you have a serious or long term medical condition or you hold a class MC licence (road train or B-double multi-combination), you need to have a regular medical review.

Transport for NSW has improved this medical review process so that all GPs, specialists, occupational therapists and optometrists can now submit NSW Fitness to Drive assessment forms for you online.

Transport for NSW has developed 12 new online specialist forms, and improved the online form for GPs, making the assessment process quicker and easier for Heavy Vehicle Drivers.

When the form is completed online it is instantly sent to Transport, saving you a trip to a Service NSW Centre to hand in the paper form.

Your doctor will get an instant response confirming your assessment has been received and a copy of the completed form will be stored for future reference.

So if you receive a request for a medical review in the mail, take the letter to your doctor and ask them to submit the assessment online for you.
